However, I'm pretty sure it will be borked in practical cases during reboot and migration, for eg.

However, I also take Vladik's point about serial specifically. From LibvirtBaseVolumeDriver.get_config:

 conf.serial = connection_info.get('serial')

So what we're doing here is adding a local device, and then matching against the local device that we just created based on a key which was hopefully created correctly in a driver-dependent opaque data structure of an external service.i.e. If future cinder ever starts sending connection_info with serial != volume id we're going to break here in a really obtuse way. That doesn't sound like a good idea, either.

I think _build_device_metadata needs a rethink. If the goal is to map tags to guest hardware addressing, we need to do this every time guest hardware addressing can change, which is every time we rewrite the domain xml. I think we need to run it on reboot, cold migration, and possibly live migration too. And potentially resume from sleep. Go audit calls to _create_domain_and_network.

Given that we should be populating device metadata in the same operation as creating the domain xml in all these cases, we should be able to find a robust and unambiguous way to map them. For example, we should be able to simply use the order they appear in the domain xml.

For attach_volume and attach_interface we're currently recreating all device metadata, even though we're only touching a single device. This seems inefficient and potentially error prone. We should look into how to touch only the device we just added. Again, I'm pretty sure that as we just added we can probably safely use the order it appears in the domain xml. I expect it's last, but you should check.

Incidentally, I don't immediately see where we're cleaning up metadata from devices that we've removed.
We can do that in compute manager without hitting the drivers [1]

[1] https://review.openstack.org/#/c/435082/11/nova/compute/manager.py
Totally unrelated, but it's weird that we handle the libvirtError to raise DeviceIsBusy when nothing is handling that specific exception in the upper layers.
